Additional Nursing Support
If you have a long-term condition such as diabetes, high blood pressure (hypertension), or another health condition that requires regular monitoring, you may be contacted by one of our community nurses.

In some cases, you may also be referred to a specialist nurse or healthcare professional, such as a diabetes specialist nurse or a dietician, who can provide tailored advice and support to help you manage your condition.

Our aim is to make sure you have the right care, from the right professional, at the right time.
