Self Referral

Most referrals to other specialists will require you seeing or speaking with a GP. However, there are a number of local services, listed below, that you are able to make self referrals to.

Young Hackney – Free Condoms

If you’re under 25 years old and live in the City of London or Hackney, you can access free condoms and lubricant from a range of local venues, which include pharmacies, GP practices, youth hubs and colleges.

First Contact Physiotherapist

Our First Contact Physiotherapy (FCP) Service in now live. This service will allow patients to receive input from an expert in the first instance. First contact physiotherapists are advanced physiotherapists who are clinically competent to manage and treat musculoskeletal problems which means you may not need to see a GP. Please book via reception.

Sexual Health London

SHL.UK is a discreet sexual health service for Londoners. Our STI testing service is for people who have mild or no STI symptoms, and would like to get themselves checked. You can also request free regular and emergency contraception (effective up to 5 days after unprotected sex) by completing a simple, secure online consultation

City and Hackney Carers Centre

There are an estimated 18,000 unpaid carers who live or support someone living in the City of London and Hackney and as such carers are at the heart of all that we that do. Our services continue to be designed, developed and closely monitored by our team, alongside our service users. Male Isolation Group – The Get Together

weekly men’s group open to all male patients registered with the Shoreditch Park and City PCN. This group will act as a drop-in session for men who want to sit down and have a chat with free tea/ coffee and biscuits. This is not a support group in the conventional sense however the aim is for them to feel supported and stay connected through social engagement.
